Is there an incorrect name, passport photo or date of birth on your card? In this case you can apply for a replacement card.
Apply for a replacement card
Download and print the form. Complete it and send it in with your old OV-chipkaart. You will receive a replacement OV-chipkaart mostly within two weeks.*

If there is more than €0 credit on your card, this amount will be returned to your bank account. You will not be charged an administration fee.
Travel products transferred
If there was a travel product or subscription on your old card, this will also be on your new card and you can simply start travelling again, as long as the travel product is still valid.

Add the new card to your My OV-chipkaart account to view the travel history for this card.
