Forms personal card

Apply for automatic reloading with international IBAN You want to apply for automatic reloading with an international IBAN. Application form auto reload international IBAN (pdf)
End card deceased card holder You want to terminate a personal card belonging to a deceased cardholder. End personal card and request a credit refund (Dutch, pdf)
(Almost) expired card You wish to renew your (almost) expired card. Renew expired card (Dutch, pdf)
Card not working Your card is broken and you want a new one. Apply for a replacement card (Dutch, pdf)
Change personal details You have moved house or your details have changed. You do not receive a new card. Change personal details for a personal OV-chipkaart (Dutch, pdf)
Credit refund You wish to terminate your personal card and you would like a credit refund. End personal card and request a credit refund (Dutch, new window)
Card expired more than a year ago You wish to request a credit refund for a card that expired more than a year ago. Request a credit refund (pdf, Dutch)
Wrong details on card The personal details on your card are incorrect (name, photograph or date of birth). You want a replacement (new) card. Apply for a replacement card (Dutch, pdf)
Change automatic reloading You want to change a bank account number or amount or you want to terminate automatic reloading. Change automatic reloading (Dutch, pdf)

Forms anonymous card

Anonymous OV-chipkaart Reason Form
Card not working Go to a counter to check whether your card is definitely broken. It may be that something else is happening and that this can be resolved at the counter. Request a credit refund (pdf)
Tip: write down your card number. You need this number if you have a question about your credit refund.
(Almost) expired card Your anonymous card is no longer valid or you want to terminate your card. Request a credit refund (Dutch, new window)
